Allow user to select fields for Ident and Comment field when loading file from ArcMap

When loading from ArcMap, the user no longer has the ability to select the fields to use for the Ident and Comment field as DNR Garmin allowed in the past. Is important to allow user to load their own values for the ident and comment fields.

Sounds good. In the mean time, if you set the field Alias in DNRGPS to match the field name in ArcMap it should pull the data through properly.
