jack midi : restart driver creates a new midi port
1) make sure you have jack midi enabled (in QjackCtl you see a port named 'hydrogen-midi' on the Midi tab)
2) switch on/off the 'create per instrument outputs' (h2 will ask if the audio engine should be restarted > click 'OK')
3) in QjackCtl (on the Midi tab) you will see that hydrogen has now 2 midi ports (both in and out) 'hydrogen-midi' and 'hydrogen-midi-01'
4) repeat step 2 and another midi port will be added ('hydrogen-midi-02' ...)
if you do the same in ALSA midi mode this does not happen

This should not happen after fixing #242. But for the numbering changes, i get a new port with hydrogen-midi-01 instead of hydrogen-midi. This is could be a problem with unregistering a port and registering it in a very short interval. Do you see the same effect?
