ldap user bind attribute names should be overrideable

I was using the lift-ldap module to hook into the ldap interface of ActiveDirectory. Everything worked great, except when binding the attributes for the cn and uid. ActiveDirectory seems to use something called "sAMAcountName" instead of "uid". Attached is a patch that extracts the attribute keys
